I had the same issue, and I resolved it by replacing one of my USB cables .

I can say I've had similar glitches with Voicemeeter. I like the program a lot, but I've uninstalled it for its random flakiness.

Could the fan be drawing enough current to be giving the problem? Maybe try to disconnect the fan and see if it works properly then.
